A company is looking for a Cardiac Rehab Intake Specialist, Contractor.
Key Responsibilities

Conduct initial assessments for patients enrolling in the virtual cardiac recovery program
Educate patients on program benefits and risk factor modification
Coach individuals through electronic messaging and video sessions

Required Qualifications

Minimum of three years of experience in health management, ideally in a clinical or remote health coaching setting
Degree in a health-related field
Experience in counseling or education of disease management
Basic understanding of cardiac procedures and medications
Some knowledge of cardiac rehabilitation
