looking for a dedicated and compassionate Case Manager to join our
team. In this role, you will manage a caseload of clients, working
directly with them to manage any legal, medical, financial needs,
etc., while providing individual and group counseling, guiding them
through their recovery journey, and helping them build a brighter
future. Youll be an integral part of a supportive and collaborative

field, this is the perfect opportunity for you! ESSENTIAL DUTIES
AND RESPONSIBILITIES Orient client to program and introduces them
to client workbook explaining assignments and due dates so they
fully understand the program and what a successful completion is.
(When we have a client workbook and discharge date in place)
Maintain confidentiality of records relating to clients' treatment
Prepare and maintain all required treatment records and reports.
Counsel clients individually and in group sessions, to assist in
overcoming dependencies, maladjustments, relapse prevention,
adjusting to life, and making changes Develop client Treatment
Plans Collaborate with other staff members to perform clinical
assessments, attending Treatment Team Staff meetings Collect
information about clients through interviews, observation, and
tests Act as (literal) client advocate to coordinate required
services or to resolve emergency problems in crisis situations
Ongoing evaluation of clients' physical or mental condition based
on review of client Meet with families, and other interested
parties to exchange necessary information during the treatment
process Refer patients, clients, or family members to community
resources or to specialists as necessary Counsel family members to
assist them in understanding, dealing with, and supporting clients.
Modify treatment activities and approaches as needed to comply with
changes in clients' status Document interactions with client using
Kipu or another compliant EMR Secure a Release of Information for
the clients family, friends, sponsor anyone who will be involved in
clients treatment Oversees coordination of needs and work in
conjunction with client and individual therapist in treatment
planning and goals, identifying additional needed support services
including educational, vocational, legal, recreational, mental
health and social services Coordinate with client and individual
therapist to create an aftercare plan, beginning as soon as
possible upon admission Assist in identifying resources and
facilitating appropriate services for referral, in conjunction with
clinical team Complete and maintain all documentation of services
delivered, which includes individual records, progress notes, group
notes, aftercare plans, etc. Ensure cohesion of services documented
within the medical and clinical charts and assists with compliance
in facility and licensing standards Complies with local, state and
federal confidentiality laws and regulations Provide crisis
intervention and clinical case management services to participants
Link clients with appropriate medical, mental health, substance
abuse, and vocational resources Provide opportunities for clients
to build a supportive peer community Facilitate life skills and
coping skills workshops and support groups JOB REQUIREMENTS
Education: Minimum of a High School Diploma or GED, (Bachelors
Degree preferred) Licensure: Maintains current registration,
certification, or license from a DHCS-approved certifying
organization (i.e. CCAPP, CAADE, CADTP) Experience: 2 years of
previous chemical dependency treatment experience in addiction
field preferred Medical Requirements: Must have a TB screen, Health
Screening, CPR/First Aid Certificate within 7 days of hire
Knowledge/Abilities: Must have the ability to monitor appropriate
documentation PHYSICAL DEMAND/WORK ENVIRONMENT Physical Effort:
Work activities require long periods of standing and sitting;
independent body mobility (stoop, bend, walk routinely to perform
daily tasks; ability to lift and carry up to 25 lbs.; sufficient
vision to read standard text; manual dexterity to use hands, arms,
shoulder repetitively to write, record and document information
daily. Stress: Performs in high stress environment with psychiatric
or chemically dependent clients. Work activities are partially
structured and require judgment in setting priorities. Clinical
decisions require judgment based upon commonly accepted principles
of recovery. Other decisions are guided by policies/procedures or
guidelines and precedents established by similar situations. Must
be physically and mentally able to assist in an emergency KEY
