Job Description:

MindPath Care Centers, a part of Community Psychiatry, is seeking a dedicated Psychiatric Mental Health Nurse Practitioner to join our growing team focused on providing comprehensive outpatient mental health services. We emphasize a personalized approach to care, integrating therapy and medication management to support patient well-being. The successful candidate will work collaboratively with our multidisciplinary team to assess psychiatric conditions, develop treatment plans, and manage patient care. This role requires excellent clinical skills and a compassionate approach to mental health care, ensuring each patient receives the highest standard of care in a supportive environment. Assess and diagnose psychiatric and mental health conditions in patients. Develop and implement treatment plans including pharmacologic and non-pharmacologic interventions. Collaborate with psychiatrists, psychologists, therapists, and other healthcare professionals to ensure comprehensive care. Monitor patient progress and modify treatment plans as necessary. Provide psychoeducation to patients and their families. Participate in team meetings and contribute to treatment planning discussions. Maintain accurate and confidential patient records according to privacy laws and standards.
