Job Description:
A company is looking for a Care Coach II to support family
caregivers and their care recipients.
Key Responsibilities
Engage and educate caregivers through phone and digital technology
to improve their wellbeing
Manage a caseload and assess caregiver and care recipient needs to
create and achieve care plan goals
Document coaching activities and collaborate with stakeholders to
identify problems and solutions
Required Qualifications and Education
4+ years of experience in care planning or case management
Experience working with the elderly or adults with disabilities
Solid understanding of care planning and case management
principles
Proven experience accessing community resources
Ability to navigate mobile applications and multitask in a
fast-paced environment
