Job Description:
A company is looking for a Telephonic Care Navigator - National
Remote.
Key Responsibilities:
Conduct telephonic outreach to members for post ER visit
assessments
Complete telephonic assessments with members
Facilitate follow-up for members and perform other assigned
duties
Required Qualifications:
High School Diploma / GED or equivalent work experience
Must be 18 years of age or older
2+ years of experience in a physician's office, SNF, ALF, or
outpatient clinic
Experience working in cross-functional, inter- and
multi-disciplinary teams
Proficiency with EMRs and knowledge of medical terminology
